We will accept referrals from any source, including self-referrals, providing the person referred is resident in Blackpool. Our preference is that referrals are received on our referral form which can be downloaded from the documents section below. Completed forms can be emailed to us at bflreferrals@addaction.org.uk or dropped off at our offices.

We aim to ensure that any referrals are dealt with as quickly andtransparently as possible. Referrals will be acknowledged within 24 hours of receipt and referrers will be contacted within 48 hours to discuss the referral and eligibility. Following this stage, an assessment of the referred individual will take place and feedback provided. 

 

Referral Criteria & Assessment

Blackpool Fulfilling Lives is targeted at people living very chaotic lifestyles who don’t currently engage with services.

Individuals referred should:

  • Present with at least two out of the four specified areas of multiple need (i.e. homelessness, substance misuse, mental ill health, reoffending)
     
  • Be over 18 years of age
     
  • Score sufficiently highly on the programme assessment

All referrals are assessed and scored using the New Directions Team (NDT) Assessment adapted to include criteria specific to the Blackpool programme.
